Substitutes

Dear Applicant:

Thank you for your interest concerning placement on our Substitute List. You will need to submit the following information for our records (some or all of which are attached):

  1. Staff Information Card
  2. Completed Employment Application. This must be typed or done online and printed out.
  3. Official Transcripts. If you have permanent teaching certification, copies are fine.
  4. Copy of New Jersey Certification (Permanent or Substitute) Apply online at https://njdoe.my.site.com/manage/s/  This applies to sub nurses as well.
  5. Oath of Allegiance
  6. Proof of Mantoux Tuberculin Test - Our school nurses will administer, if necessary.
  7. W-4 Forms. Included are W-4, State Withholding, NJCSESP, I-9, and Direct Deposit. Please include either your Passport, or your Driver's License, and Social Security card.
  8. Criminal History Review (Fingerprint Process) go to https://www.nj.gov/education/crimhist/ to pay and make an appointment.
  9. Sexual Misconduct Form - the first and second pages need to be filled out by the applicant then forwarded on to any employer that the applicant has worked for in the last 20 years. The employer will then return the form to dguardino@pascack.org.

  10. Must complete (yearly) Harassment, Intimidation, and Bullying Online Training Course.

If you do not have a NJ Teaching Certification, you will need to apply for a Bergen County Substitute Certificate online. See #4 above.  You will need official transcripts from your college/university showing at least 60 college credits sent directly from your email to the state, and a Fingerprint Approval letter from the Criminal History Review Unit to finish your application.
